Free survey readiness self-check for Florida ALFs

A free 5-minute self-check for Florida ALF and memory care operators: about 15 questions on your processes, a status per area, and an emailed action plan.

Last updated

Answer about 15 multiple-choice questions on how your facility runs, and see where you stand before your next AHCA survey. It takes about 5 minutes.

You get a status for each area straight away. If you'd like, we then email you an action plan, timed against Florida's new Memory Care Services license: AHCA's rules are due by June 1, 2027, and facilities already licensed when the rules take effect will have six months after that date to get the license.

The self-check asks about your processes, never about residents. Please don't enter any resident or health information.

It is a self-assessment to help you prioritize, not legal advice, and it can't predict what a surveyor will cite.

The self-check covers:

  • About your community
  • Resident records and health assessments
  • Medication practices
  • Staffing and training
  • Background screening
  • Incidents and emergency planning
  • Memory care readiness

If the self-check doesn't load, please refresh the page or request early access.
